Designer looking for a Programmer for 1 job

  I have a fairly complicated script on a client's site that has just stopped working. It no longer writes to the database and it doesn't pass a value correctly to a second page. I have another form that - other than the fact that it also no longer writes to the db - works as the broken form should.
  
  Summary of what I need done: Get both forms writing to the db again and have the main form pass the value correctly to the second page.
